Getting a new technician started in Iowa
Iowa Medicaid does not require RBT certification for technicians who hold a bachelor's degree, so a degreed BT may start under licensed supervision while pursuing RBT; claims must be submitted by the supervising licensed behavior analyst. Iowa's Autism Support Program (separate from Medicaid, ages under 14, income up to 500% FPL) funds ABA for children without Medicaid or private ABA coverage. (Our research note; confirm with the agencies on the BT vs RBT page.)
What we are still checking
- Exact degree wording in 441 IAC 77.26(10) (any bachelor's degree vs. listed fields).
- Whether ABA procedure codes are on Iowa Medicaid's approved telehealth list and any ABA-specific in-person requirements.
- Telesupervision rules for RBTs under Medicaid and commercial plans.
- Whether Molina Healthcare of Iowa and Wellpoint Iowa cover ABA and their RBT/BT requirements.
- Exact current name of the licensing board and licensure fees/timeline.
- Whether self-funded plans are in scope of 514C.31 (statute is silent).
- Background check, fingerprinting, CPR or TB requirements for BTs in Iowa (not found).
- Whether an unregistered BT may bill under commercial insurance and any window to obtain RBT.
- autism_insurance_mandate: verifier astra marked conflict - The 2025 age restriction, three dollar tiers, and exclusion of non-ABA payments from those tiers are historically supported, but obsolete for current guidance. HF330, 2025 Acts chapter 162, removed the age restriction, annual-cap subsection, and visit-limit authorization for specified policies delivered, issued for delivery, continued, or renewed on or after January 1, 2026. Section 514C.22 additionally prohibits ASD annual/lifetime limits and specified outpatient visit limits. The employer threshold, preceding-year 50%-of-working-days test, non-state public-employee scope, and physician/psychologist/licensed-behavior-analyst practitioner categories remain supported. Individual-policy exclusions are now subsection 8, not 9. Prior authorization remains permitted. The three-month/ six-month review schedule permits more frequent review when the carrier and treating physician or psychologist execute an individual-specific agreement. The original January 1, 2018 applicability date and January 1, 2023 definitional-amendment applicability are supported. Keep self-funded-plan applicability unresolved from this section alone; statutory silence does not establish an ERISA conclusion.
- medicaid_aba: verifier astra marked conflict - Substantive coverage, qualifications, supervision, and billing claims remain supported, but replace 441—77.26 with 441—77.31(8)–(10), effective July 1, 2026. Assistants must themselves be licensed, hold current qualifying certification, and work under a licensed behavior analyst; certification alone is insufficient. Technicians qualify through current BACB RBT certification OR any bachelor's degree. Only the supervising licensed behavior analyst submits their claims. No supervision percentage or hour requirement appears in this provision. The current assistant subsection inconsistently names the board of psychology; Iowa Code 154D.1 and DIAL identify the Board of Behavioral Health Professionals. Hawki exclusion is supported both historically and by the 2026 handbook. The HHS agency identification and enrollment URL are supported.
- bcba_licensure: verifier astra marked conflict - Replace superseded 645—31.9 with 481—880.2(2). Set the board to Iowa Board of Behavioral Health Professionals and board_url to https://dial.iowa.gov/about-dial/boards-and-commissions/board-behavioral-health. The license titles, 2018 enactment, BACB credential pathway, and assistant supervision requirement are supported. Include BCBA-D alongside BCBA; assistant applicants use BCaBA certification. A completed application and nonrefundable $120 fee are also required. Section 154D.2A uses the broader statutory term certifying entity, defined in 154D.1 to include BACB or another appropriately accredited entity; the current administrative rule specifically says BACB. Qualify state_license_required with statutory exemptions: 154D.4(3)(i) includes analysts licensed elsewhere or certified to practice independently who work in Iowa no more than 2,080 hours per calendar year. Other exemptions include supervised technicians and specified school personnel. Do not present licensure as universally mandatory without these qualifications.
- telehealth_aba: verifier astra marked conflict - Replace unknown with conditional yes for Iowa Medicaid: the official July 10, 2026 list permits codes 97151–97158 through audio/video with modifier 95, effective January 1, 2024; these rows do not permit audio-only or inpatient telemedicine. This is not blanket authorization for every ABA activity or payer. Replace 441—78.55 with 441—78.54, effective July 1, 2026; current 78.55 concerns community-based neurobehavioral rehabilitation. The current telehealth rule retains the no-prior-in-person-contact provision and reimbursement equivalence, subject to professional standards and 481—655.9. The 514C.34 coverage parity, mental-health payment parity, January 1, 2019 applicability, and excluded modalities remain supported. Its definition includes other real-time interactive electronic media in addition to audio/video. The 2025 text separately identifies January 1, 2021 applicability for the mental-health amendments. Remote credential supervision and additional ABA-specific in-person requirements remain unverified; permission to bill 97155 remotely does not alone establish every supervision requirement.
- bt_vs_rbt: verifier astra marked conflict - The substantive conditional conclusion is supported, but the current Medicaid citation is 441—77.31(10), effective July 1, 2026. Any bachelor's degree satisfies its degree alternative; the text does not restrict the major or add a 40-hour training requirement to that alternative. Replace 'the supervisor billing' with 'the supervising licensed behavior analyst submits claims'; an assistant may supervise treatment but is not the designated submitting professional. The 2022 agency response expressly rejected a requested 45–60-day billing window for staff lacking a bachelor's degree while obtaining RBT certification. Add the 154D.4(3)(c) supervision and nonprofessional-title conditions to state_specific_bt_requirements. Medicaid eligibility does not confer BACB RBT certification or establish commercial-payer eligibility.
