Getting a new technician started in Illinois
In Illinois Medicaid the RBT credential (and age 21+) is needed before delivering ABA; a new BT should complete the BACB RBT process first. Employment through a Behavioral Health Clinic or an arrangement with a supervising BCBA plus clinical supervisor is needed. (Our research note; confirm with the agencies on the BT vs RBT page.)
What we are still checking
- Illinois behavior analyst licensing act citation, titles, fees, implementation and exemptions (IDFPR).
- Telehealth rules for ABA delivery and telesupervision (Medicaid and commercial).
- Medicaid managed care plans (HealthChoice Illinois MCOs) and their ABA requirements.
- IMPACT enrollment URL and steps.
- Background check, fingerprinting, CPR and health screening requirements for BTs.
- Whether the Oct 2020 notice's description of BCBAs as licensed clinical psychologists/LCSWs is still current given licensure.
- Whether self-funded plans are excluded from 356z.14 (statute text silent); whether HB5225 was signed and in effect.
- Current HFS handbook/rule (89 Ill. Adm. Code) text for ABA.
- Whether HFS and each Medicaid managed care plan expressly accept live-video supervision as satisfying the ABS rule's face-to-face contacts; BACB permits video supervision, but Illinois Medicaid acceptance and separate supervision reimbursement were not established.
- Telehealth coverage for technician treatment codes 97153 and 97154, commercial-plan ABA telehealth code and credential requirements, and plan-specific authorization and reimbursement rules remain unverified.
- Current HFS handbook implementation and reconciliation with older notices remain unverified. The adopted ABS rule effective 12/27/2023 was read for this telehealth pass.
- The older age-21 and dual-oversight descriptions retained outside telehealth require reconciliation with 89 Ill. Adm. Code 140.465: the adopted rule lists BACB-certified RBTs without a separate age-21 condition, sets age 18 for developmental-intervention technicians, and permits RBT supervision by an ABS Clinician credentialed as LCP, BCBA, BCBA-D, or BCaBA.
- medicaid_aba: verifier astra marked conflict - Coverage, HFS administration, program classification, historical start date and cited SPA effective date are supported. Provider and supervision statements are outdated. Current 89 Ill. Adm. Code 140.465 recognizes LCPs, BCBA/BCBA-Ds, supervised BCaBAs and specified developmental-intervention clinicians. RBT supervision may come from an appropriately credentialed LCP, BCBA, BCBA-D or BCaBA; the asserted universal LCP/LCSW-plus-BCBA arrangement is not current. The rule retains 5% monthly supervision and two contacts, but contains no eight-RBT ceiling. That ceiling concerned collaborative agreements in the superseded 2020 notice. Remove the blanket age-21 requirement; see bt_vs_rbt. TN 21-0018 states age 21 for its ABS Technician category, not for ABS Therapists or RBTs. The supervising-BCBA claim-field requirement is verified historically, not independently established as current for every arrangement. IMPACT enrollment is supported; populate its URL with https://hfs.illinois.gov/impact.html. [Current ABS rule](https://www.ilga.gov/commission/jcar/admincode/089/089001400D04650R.html).
- bt_vs_rbt: verifier astra marked conflict - Replace age 21 as a current Medicaid RBT requirement. Section 140.465(b)(2)(A) now uses BACB certification and good standing; BACB eligibility begins at 18 with secondary education or its equivalent. The current developmental-intervention technician alternative expressly uses age 18, not 21. Also update terminology: ABS Technician is now an umbrella category containing RBTs and qualifying developmental-intervention technicians. Section 140.465(c)(2)(B) still requires RBT credentials for technicians delivering Medicaid comprehensive/focused ABA. Scope the 'no' field to that reimbursement rule; it is not a statewide prohibition on all supervised unregistered BT work. Section 20(c)(2) provides a separate licensure exemption, without establishing commercial reimbursement. A notice updated January 30, 2026 proposes enrollment while seeking certification, but no opened approval established an operative grace period or its duration. Do not invent one or claim its universal absence. [Current Medicaid rule](https://www.ilga.gov/commission/jcar/admincode/089/089001400D04650R.html).
